Lethargy and confusion

A 71 year old female with a history of high blood pressure and type 2 diabetes, presents to the Emergency Department for lethargy and confusion. She was found by her daughter this morning to be very tired and slow to respond. On arrival by emergency medical services (EMS/Ambulance), the patient's vitals were, HR: 101, BP 86/42, RR 24, SpO2: 93% on room air, Temperature 101 F (38.3 C). In the emergency department, she was found to have a blood work significant of a WBC 17K and her urine showed +leukocyte esterase and +nitrites and many WBCs.
